Client Forms

Our Client Forms page is designed to provide you with easy ways to document the information we will need to help you, as well as find other information important to you. 

In the Fee Disclosure Forms by State section, you will find the latest version of our Form ADV Part II.  All Registered Investment Advisers in the State of Texas are required to provide this form, or a brochure containing equivalent information, to all clients. 

The Questionnaires section provides you with our key required or optional forms.  How do you know which forms to use?  Follow these simple guidelines:

  1. For those comfortable using online data tools (our Preferred Approach):
    1. The only form you need to download from our Questionnaires section is the Comprehensive Confidential Questionnaire
    2. Once you have downloaded the Comprehensive Confidential Questionnaire, print it, complete it, and bring it with you to the Get Acquainted Meeting. 
    3. Wait to receive an email from GA Bishop and Associates directing you to a website where you can complete our Risk Assessment

  2. For those of you uncomfortable with online data tools:
    1. Download the Risk Questionnaire - Paper Form.
    2. Download the Comprehensive Confidential Questionnaire.
    3. Or, call us at (972) 393-3716 to have the forms mailed to you.

  • Comprehensive Confidential Questionnaire

    All clients must complete this form and bring it with them to the Get Acquainted Meeting.

  • Risk Questionnaire - Paper Form (Optional)

    This questionnaire is only necessary if you do not use our online risk assessment tool. Our Risk Assessment is optional for the Get Acquainted Meeting, but required once you become a client. If you are married or have a significant other who will be a part of the engagement, both must complete a separate assessment.
